For your convenience, you may now select a Novartis Oncology Service Request Form (SRF) that is customized to fit your patient’s needs. This new, streamlined SRF includes only those fields relevant to your patient’s medication.
A complete submission will include a separate SRF that is completed by your patient. Encourage your patient to provide consent by submitting his or her SRF first.
Patients have 2 options to submit their SRF. They may do so online (preferably) or download the SRF to print and fax. Currently, health care providers (HCPs) may only submit their SRF via fax.
Expect a call and/or fax from Novartis Patient Assistance Now Oncology within 24 to 48 hours of a complete submission.
Choose your product to generate a custom form, then submit via fax.
Direct your patients to the Novartis Oncology patient website. There, they will have the option either to complete the form online or download it to print and fax.

At Novartis Oncology, we've made it easy for patients to access available financial support for their prescription costs for Novartis Oncology medications. With the Novartis Oncology Universal Co-pay Program, eligible patients with private insurance may pay $25 per month and Novartis will pay the remaining co-pay, up to $15,000 per calendar year.*
