- A combination of PANO case managers and/or field reimbursement managers are available to help, depending on the complexity of a patient’s case
Please note: Requests for support from the Novartis Patient Assistance Foundation, Inc. (NPAF), an independent organization, must also go through PANO.
How to Get Started with PANO
How to Get Started with PANO
For a patient to access these cost-saving programs, a service request form (SRF) must be filled out and submitted. The submission process for the form — which includes a patient half and an HCP half — is designed to be flexible for both parties. This form can be submitted online or by faxing to PANO at 1-888-891-4924.
Step 1: Patient Submits Form
A patient must complete and submit their half of the SRF, after which they will receive a confirmation number.
Step 2: HCP Submits Form
Your office can submit your half of the SRF online or by fax. If your office is submitting online, the patient must submit their half first. You will need the following:
- The patient's confirmation number and date of birth. The confirmation number can be obtained by calling PANO at 1‑800‑282‑7630.
- A digital signature of the prescribing HCP is not required for online submission. Instead of having the HCP digitally sign, a member of the office staff authorized by the HCP may digitally attest to the accuracy of the information on the SRF.
- The prescribing HCP still needs to submit a prescription.
The HCP may also print, complete, and sign the entire SRF, including the prescription, and fax to PANO at 1‑888‑891‑4924.
Step 3: PANO Processes Form
PANO will then match the 2 halves and reach out to your patient and office with next steps.
